WAIVER & RELEASE OF LIABILITY

Portland Indoor Soccer, Inc. is furnishing only the opportunity to use an indoor athletic facility, and is not providing either supervision or instruction. I understand that participation in indoor soccer can be dangerous, including the possibility of serious and permanent bodily injury. In consideration of the right to use this indoor soccer facility, I agree to assume the above risks, and release and hold harmless Portland Indoor Soccer, Inc., and all others having any interest in this facility from all liability, negligence, causes of action, claims, demands, and damages of every kind which may arise out of my participation in activities at this facility. I have played soccer before, and understand the rules of the game and the above risks. I will play under control within the rules of the game, and to the best of my ability will avoid causing injury to myself and other persons using this facility.
